"COPY & CREDIT PROVIDED" FORM

INSTRUCTIONS:

Please note that there are 4 pages total. The first Agreement Form (first 2 pages) is labeled "Performer Copy" and the other Agreement Form (last 2 pages) is labeled "Contractor Copy". 

You, the Performer, will fill-out your information on the"Contractor Copy", sign it and leave the Form with the appropriate person on the production staff.

Then have the appropriate person on the production staff fill and sign the "Performer Copy" and give back to you. Make sure all of the fields are properly filled-out then store your "Performer Copy" in a safe place.

Please feel free to make copies of these Agreement Forms - use and distribute them to other performers whenever the need arises!
